UCR Health offers primary care services for adults with developmental disabilities. Our physicians are board certified in their specialty and are dedicated to improving the health of their patients. Care is provided at the Access Clinic located at Riverside University Health System in Moreno Valley.

A fundamental goal of our practice is to coordinate medical care between adult disabled patients, their family and caregivers, and their primary care physician and specialists. The practice uses the “patient-centered medical home” model to improve accessibility to care and health outcomes for their patients.

Primary care doctors treat a wide range of conditions, including:
  • Acute illnesses, such as colds, flu and urinary tract infections
  • Age-related conditions, such as dementia and Alzheimer’s disease
  • Chronic conditions, such as asthma, diabetes, obesity and high blood pressure
  • Common childhood and adolescent illnesses, such as ear infections, pink eye and strep throat
  • Chronic pain associated with arthritis or injury

  • Mental health services, including Alzheimer’s disease and dementia care
  • Pain management
  • Palliative care to relieve pain and symptoms caused by serious illnesses
  • Physical exams for school, day programs and special Olympics
  • Preventive care, including immunizations, well check-ups and screenings
  • Routine follow-up for heart disease, high blood pressure and other cardiac conditions
  • Same day care for acute illnesses
  • Social support and assistance with completion of forms
