How to Switch Your Care to UCR Health

If you're ready to switch your care to UCR Health, it's easy to get started. We'll help you every step of the way.

If you're selecting a new health plan during your annual Open Enrollment period or after a qualifying life event — such as a change in residence, job situation, or marital status, or becoming eligible for Medicare at age 65 — make sure the plan includes UCR Health's expanding network of providers. Then you can register as a new patient and receive award-winning care from our nationally recognized doctors and advanced specialists.

It's often possible to switch between open enrollment periods, too. Depending on your health insurance plan, you may be able to access UCR Health at any time by switching to one of our primary care providers (PCPs).

How to Switch Your Care: Step-by-Step Guide

Health insurance through an employer

  1. Check Insurance Plans Accepted at UCR Health to see if we accept your current plan. If so, you may be able to change to a UCR Health primary care provider (PCP) at any time.
  2. To select a new plan through your employer, call the health plan directly to verify if UCR Health is in the network. They may ask you for your employer information and our Tax ID numbers, which are Medical Group Tax ID # [number] and Medical Center Tax ID #[number]. (DOUBLE-CHECK THE TAX ID INFO).
  3. Have other questions? Call our concierge line at 855-827-4264.
  4. Did you switch your plan? Call us at 844-827-8000 to update your insurance information.

Private commercial insurance

  1. Check Insurance Plans Accepted at UCR Health to see if we accept your current plan. If so, you may be able to change to a UCR Health primary care provider (PCP) at any time.
  2.  If you are selecting a new plan from the list above, call the health plan directly to verify if UCR Health is in the network. Medicare

  1. Check Medicare Plans Accepted at UCR Health to see if we accept your current plan. If so, you may be able to change to a UCR Health primary care provider (PCP) at any time.

UC employee insurance

  1. Current UC employees: You can choose between UC Blue & Gold HMO or UC Care PPO to receive care at UCR Health.
    • UC Blue & Gold plan: After you select this plan in UC Path, you will be asked to choose a primary care provider. You do not need to designate a certain provider. Do not click “Select a Provider” next to the field; instead, after reviewing the checkboxes below, choose “Save and Continue.”
    • UC Core: This plan is administered by Anthem Blue Cross. There’s no need to choose a primary care physician. Your most optimal benefits will be applied when you see a UCR Health provider or other Tier One provider.
  2. UC retirees: Please call 844-827-8000 for more information.

MyUCRHealth securely houses your electronic medical record online, making it portable and accessible from anywhere you go.

What is MyUCRHealth?

Does UCR Health have its own laboratory services?

We do not currently offer on-site lab services, but we do partner with regional laboratory providers for convenient and accessible services.

Does UCR Health have its own pharmacies?

We do not currently offer our own pharmacies, however we do partner with several pharmacy locations.
